Pulse 82 bpm, regular. Clear chest. Pedal pulses present. Foot examination normal with intact sensations.

Uncontrolled Type 2 Diabetes Mellitus with Stage 1 Essential Hypertension

Advised low glycemic index diet, daily 30 min brisk walk, monitor fasting sugar weekly, and follow up with HbA1c.

Patient has 4-year history of diabetes. Complains of throat tickle and feverishness for 2 days. Fasting blood sugar this morning was 134 mg/dL. No breathlessness or chest tightness.

Pulse 78 bpm, BP 130/84 mmHg, Temp 99.1°F, SpO2 99%. Pharyngeal congestion present, tonsils not enlarged. Bilateral vesicular breath sounds, no added sounds.

Acute Pharyngitis with stable Type 2 Diabetes Mellitus & Controlled Hypertension

Advised warm saline gargles TDS, adequate hydration, strict avoidance of cold beverages. Continue regular Metformin 500mg SR and Telmisartan 40mg. Follow up in 5 days if fever persists.
